Office location negotiable depending on location of applicant and availability of space in the team coverage area. The Bureau of Special Health Care Needs is seeking a qualified, compassionate, and dedicated candidate to fill a Registered Nurse/Service Coordinator position located in the St. Louis area. This position will serve as service coordinator for the Healthy Children and Youth (HCY) and Medically Fragile Adult Waiver (MFAW) programs.

The salary indicated represents a base pay rate. If the individual selected or the position is eligible for a pay differential (e.g., shift, security, or years of service), it will be added to the total compensation in your paycheck. A pay differential does not raise your base pay.

Responsibilities - What you’ll do
  • Conduct home visits and complete assessments to determine medical necessity in multiple Missouri counties.
  • Document interactions in a timely and accurate manner.
  • Prior authorize medically necessary person centered in-home services.
  • Frequent travel is necessary and may require the use of a personal vehicle with mileage reimbursement. Occasional overnight travel will be required.
  • Collaborate with health care professionals and other state agencies to ensure appropriate services are in place to best meet the needs of individuals with special health care needs.
  • Advocate for and support individuals with special health care needs to achieve the best possible health and greatest degree of independence.
  • Connect individuals with special health care needs with resources to enhance the quality of their lives.
Qualifications - All you need for success Minimum Qualifications:
  • Candidates must hold a current Missouri Registered Nurse license.
  • Experience in service coordination/case management, home care/home health, and pediatrics is preferred
  • Excellent time management, computer literacy, strong interviewing and assessment skills.
  • Must possess the ability to respect, understand, communicate with, and effectively interact with diverse populations.
  • A positive attitude, exceptional organizational skills, and the ability to work well with others are strong qualifications for this role.
  • A skill test may be administered at the time of the interview.
