Madison County Office for the Aging, Inc is seeking an NY Connects Information and Assistant (I&A) Specialist. This position is the first direct contact with the community, including clients who are elderly and/or disabled, caregivers and families of, that are seeking information and access to long term services for the purpose of remaining in the community rather than institutional placement. This position will provide a single, coordinated system for accessing long term care services for all persons regardless of payor source, that will minimize confusion, enhance individual choice, and support informed decision making. This employee assists clients directly by assessing and screening clients primarily over the phone utilizing a comprehensive and current resource listing of long-term care services, programs, and providers in Madison County to support their needs.
