JOB This is professional work providing a full range of case management services in the field of drug and alcohol treatment. The employee will screen and assess their clients' needs to determine the level of care required based on placement guidelines. They will be responsible for coordinating access to treatment services and developing, and implementing a comprehensive service plan to include case management follow-up.The employee will also monitor the treatment progress of their clients, write reports, and advocate for clients with other agencies or organizations to maximize clients' benefits. Work is assigned by the Case Management Supervisor in accordance with the objectives of SCA's program and needs. Every client's case is reviewed with the Case Management Supervisor to ensure work is completed in its entirety, and in compliance with the SCA's policies and procedures.
