Assignment Details

• Shift: Day Shift

• Schedule: Monday – Friday

• Hours: 8 hours per day

• Hours per Week: 40

• Shifts per Week: 5

• Weekly Gross Pay: $2,286

• W2 Rate: $26/hour

• Weekly Stipend: $1,246

Position Summary

We are seeking an experienced Physical Therapist (PT) for a 13-week contract assignment in a Skilled Nursing Facility / Long-Term Care setting in Montour Falls, NY. The ideal candidate will deliver high-quality rehabilitative care to residents while managing a full caseload independently in a post-acute care environment.

Key Responsibilities

• Evaluate residents’ physical conditions, functional limitations, and rehabilitation needs

• Develop and implement individualized treatment plans based on clinical assessments and physician orders

• Provide therapeutic exercises, gait training, manual therapy, and functional mobility training

• Monitor patient progress and adjust treatment plans to optimize outcomes

• Document evaluations, daily treatments, progress notes, and discharge summaries accurately and timely

• Collaborate with nursing, physicians, and interdisciplinary care teams to support resident goals

• Educate residents and caregivers on treatment plans, mobility strategies, and safety precautions

• Maintain compliance with regulatory standards, safety guidelines, and infection control protocols

Required Qualifications

• Active New York State Physical Therapist (PT) License (required)

• Minimum 2+ years of Physical Therapy experience

• Ability to work independently and manage a full caseload

• Strong clinical judgment, communication, and documentation skills

Preferred Qualifications

• Experience in Skilled Nursing Facility (SNF) or Long-Term Care settings

• Previous travel experience (preferred, not required)

• Experience in post-acute or long-term care environments
