What You’ll Do

• Serve as the primary RN Case Manager for hospice patients with life-limiting illness
• Provide skilled nursing care to patients in private homes and long term care facilities.
• Manage pain control, symptom management, wound care, and medication oversight
• Support patients and families through end-of-life decision-making with clarity and compassion
• Coordinate care with physicians, social workers, chaplains, CNAs, and the interdisciplinary team
• Educate caregivers on disease progression, comfort measures, and hospice services
• Maintain accurate, timely, and compliant documentation in the EMR
• Help shape workflows and care processes as the agency continues to grow

Who Thrives in This Role

• Active Minnesota RN license
• 2+ years of experience required in hospice, oncology, palliative care, end-of-life care, or complex geriatric nursing
• Strong background in home care, assisted living, SNF, or hospital-based care
• Confident clinical decision-maker with excellent critical thinking skills
• Emotionally grounded, compassionate, and comfortable supporting patients at end of life
• Organized, self-directed, and comfortable working independently in the field
• Committed to ethical, patient-centered, and regulation-compliant practice
