MDS Coordinator

Tittle: Interim MDS Coordinator

Unit: LTC Managment

Location: Atkinson, NE

Pay and Shift Details: $3120/week for 40 hours & Primary 8-5PM

Responsibilities:
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for all residents
- Ensure accurate and timely submission of MDS assessments to regulatory agencies
- Collaborate with healthcare providers to develop individualized care plans based on MDS assessments
- Utilize electronic health record systems such as eClinicalWorks, Athenahealth, or similar platforms
- Conduct comprehensive assessments of patients' health status and needs
- Assist in discharge planning and coordination of care transitions
- Maintain knowledge of HEDIS measures and ensure compliance with quality standards
- Work closely with managed care organizations to optimize reimbursement for services provided.

Requirements:

• Must have 2 years of Experience
• BLS (AHA)
