- Salary: $109,762 – $135,000/year (based on experience)
- Incentive Bonus: Up to 17.5%
- Sign-On Bonus: $10,000

We are seeking a dynamic and experienced Director of Case Management to lead and oversee case management operations within a high-volume acute care hospital. This leadership role is responsible for ensuring effective care coordination, discharge planning, regulatory compliance, and optimal utilization of healthcare resources while improving patient outcomes.
- Provide strategic leadership and oversight of all case management operations
- Direct care coordination and discharge planning to ensure timely and efficient patient flow
- Manage departmental operations including staffing, productivity, and performance metrics
- Analyze operational data and implement performance improvement initiatives
- Ensure compliance with organizational policies, regulatory standards, and accreditation requirements
- Collaborate with executive leadership and interdisciplinary teams to align clinical and operational goals
- Optimize resource utilization and drive initiatives to reduce length of stay
- Develop and maintain effective staffing models and team competencies
- Foster a collaborative and high-performing team environment
- Lead change management initiatives and continuous improvement strategies
Education:
- Bachelor’s Degree (required)
- Master’s Degree in Nursing, Healthcare Administration, or Business (preferred)
Licensure:
- Active RN, LCSW, or LMSW license (required)
Experience:
- Minimum 3+ years of acute care hospital case management experience
- Minimum 2+ years of leadership experience in case management
- Strong experience in utilization review, discharge planning, and care coordination
- Demonstrated success in reducing length of stay and improving operational outcomes
- Strong background in acute care hospital case management
- Proven leadership experience in large, complex healthcare settings
- Data-driven with expertise in performance improvement initiatives
- Excellent stakeholder management and interdisciplinary collaboration skills
- Ability to lead teams, manage capacity, and drive measurable results
