Registered Nurse (RN) – Hospice Care Coordinator / Case Manager
Schedule: Monday through Friday, 8:00 AM – 4:30 PM
On-Call/Weekends: None
Territory: Primarily Berks County, with occasional coverage in Montgomery and Chester Counties
Mileage Reimbursement: $0.62 per mile
Position Summary
We are seeking a compassionate and experienced Registered Nurse (RN) with a background in hospice care to join our team as a Hospice Care Coordinator / Case Manager. This role is ideal for a dedicated clinician who is passionate about delivering patient-centered care, ensuring quality outcomes, and supporting patients and families through all stages of the hospice journey.
Key Responsibilities
- Perform comprehensive clinical assessments for high-risk patients to determine care needs and appropriate interventions
- Complete a full range of hospice visits, including:
- Admissions
- Recertifications
- Routine visits
- PRN (as-needed) visits
- End-of-life (death) visits
- Manage a daily caseload with a productivity expectation of 5–7 visits per day
- Develop, implement, and continuously update individualized care plans in collaboration with primary care providers and interdisciplinary teams
- Coordinate care transitions across healthcare settings to ensure continuity and optimal patient outcomes
- Collaborate with physicians, social workers, discharge planners, and other healthcare professionals to align care plans
- Identify and address barriers to care, including social determinants of health (e.g., housing, transportation, access to resources)
- Engage specialty services and community resources to support patient needs
- Monitor and document clinical, functional, and financial outcomes accurately and thoroughly
- Promote health education, illness prevention, and early intervention strategies
- Proactively identify risks and intervene to prevent adverse events whenever possible
- Serve as a patient advocate, ensuring privacy, dignity, and confidentiality at all times
Required Qualifications
- Active, unrestricted Registered Nurse (RN) license
- Valid driver’s license, reliable transportation, and current auto insurance
- Current BLS/CPR certification
- Hospice experience and Homecare Homebase (HCHB) experience required
- Associate’s or Bachelor’s degree in Nursing or related field
- Strong clinical assessment, critical thinking, and decision-making skills
- Ability to manage complex patient cases and work independently in the field
Preferred Qualifications
- Case Management certification (CCM or equivalent)
- Experience coordinating care across multiple healthcare settings
- Familiarity with cost-effective care strategies and utilization management
