Pay: $2050-$2100/wk plus .42 a mile for patient related travel

Schedule: Monday-Friday 8 AM-5 PM

Emergency On Call Only

Service Area: 60 miles Mansfield office

Requirements

Must have at least 1 year of Hospice experience within the last 3 years as an RN.

HCHB charting experience is also required.

Duties

  • Conduct comprehensive patient assessments to determine physical, emotional, and spiritual needs in hospice settings
  • Develop and implement personalized care plans aligned with patient wishes and medical requirements
  • Administer medications and treatments with precision, monitoring for effectiveness and adverse reactions
  • Collaborate closely with interdisciplinary teams including physicians, social workers, chaplains, and family members to coordinate holistic care
  • Educate patients and families about disease progression, symptom management, and end-of-life options
  • Document all patient interactions accurately in medical records while adhering to HIPAA regulations
  • Manage discharge planning and post-acute care coordination to ensure seamless transitions for patients requiring ongoing support

Requirements

  • Valid nursing license with current registration in the state of practice
  • Critical care experience including ICU or emergency room settings preferred
  • Experience working in Level I or Level II trauma centers or hospital medicine environments
  • Knowledge of infectious disease management, physiology, and complex case management procedures
  • Strong understanding of medical terminology, vital signs assessment, and diagnostic evaluation techniques
  • Familiarity with hospital records systems, discharge planning processes, and utilization management protocols
  • Excellent patient service skills with a compassionate approach to end-of-life care
  • Ability to perform patient assessments accurately in inpatient or outpatient settings
  • Experience with post-acute care environments such as hospice or home health services is advantageous
