Join our dedicated healthcare team as a 7/7 Hospice Registered Nurse on a 13-week contract, providing compassionate end-of-life care to patients in their homes and community settings. 40 hours/per week guaranteed
Service Area: 60 miles east and west of Columbia MO
Requirements
Must have at least 1 year of Hospice experience within the last 3 years as an RN.
HCHB charting experience is also required.
Responsibilities
- Deliver comprehensive patient assessments, including vital signs, symptom management, and physiological evaluations to inform personalized care plans.
- Administer medications and treatments in accordance with physician orders, ensuring safety and efficacy.
- Coordinate discharge planning and post-acute care strategies to facilitate smooth transitions between care settings.
- Conduct case management activities, including intake assessments, utilization management, and diagnostic evaluations to optimize patient outcomes.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations and hospital policies.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans that address physical, emotional, and psychosocial needs.
- Provide education to patients and families about disease progression, symptom management, and end-of-life options to foster understanding and comfort.
Requirements
- Valid RN license with current registration in the state of practice.
- Critical care experience or ICU background preferred; experience in Level I Trauma Centers or Level II Trauma Centers is highly advantageous.
- Demonstrated expertise in hospital medicine, emergency medicine, or post-acute care settings such as outpatient clinics or inpatient units.
- Knowledge of infectious disease care, geriatrics, behavioral health, medical-surgical nursing, and physiology fundamentals.
- Experience with patient assessment techniques, vital signs monitoring, discharge planning, medical records management, and diagnostic evaluation processes.
- Familiarity with HIPAA compliance standards and medical terminology essential for accurate documentation.
- Strong case management skills including intake procedures, utilization management, inpatient/outpatient coordination, and post-acute care planning.
