Job Summary
Join our dynamic healthcare team as a Home Health Registered Nurse on a 13-week contract, delivering exceptional patient-centered care in the comfort of patients' homes. This role offers an exciting opportunity to apply your diverse clinical skills, including critical care, emergency medicine, and post-acute care, to improve health outcomes and promote independence. As a vital member of our home health team, you will assess patient needs, develop personalized care plans, and coordinate services to ensure safe and effective recovery. Your expertise in hospital medicine, ICU, trauma centers, and outpatient settings will be instrumental in providing comprehensive nursing support across a variety of complex cases.

Pay: $2000/Wk plus .42 a mile for patient related travel

13 Week Contract

Schedule: Monday - Friday. Guaranteed 40 hours per week

Service Area: 3 days Indianapolis and 2 days Lafayette

Requirements

Must have at least 1 year of Home Health experience within the last 3 years as an RN.

HCHB and Oasis charting experience is also required.

Responsibilities

  • Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations to determine individualized care needs.
  • Implement and adjust care plans based on ongoing patient evaluations, ensuring alignment with best practices in acute and post-acute care.
  • Administer medications and treatments with precision while monitoring for adverse reactions or complications.
  • Coordinate discharge planning and facilitate seamless transitions between hospital, outpatient, and home settings to optimize recovery trajectories.
  • Maintain detailed medical records in compliance with HIPAA regulations and ensure accurate documentation of patient progress and interventions.
  • Engage in diagnostic evaluations and utilize medical terminology effectively to communicate findings with interdisciplinary teams.
  • Provide education on disease management, medication adherence, and lifestyle modifications to empower patients in their recovery journey.

Experience

  • Proven clinical experience in critical care environments such as ICU or Level I Trauma Centers.
  • Hands-on experience with hospital medicine, emergency room procedures, or behavioral health settings.
  • Strong background in infectious disease care, geriatrics, pediatrics (PICU), or outpatient clinic environments.
  • Familiarity with medical-surgical nursing practices and post-acute care protocols.
  • Knowledge of physiology principles, utilization management processes, case management strategies, and diagnostic evaluation techniques.
  • Prior experience working within managed care frameworks or outpatient clinics is highly desirable.
  • Demonstrated ability to perform comprehensive patient assessments and deliver high-quality patient service across inpatient and outpatient settings.
