Job Title: RN Case Manager
Location: Hyannis, Massachusetts, 02601
Schedule: 4 days/week (10-hour shifts)
Shift Options: 7:30 AM – 6:00 PM or 8:00 AM – 6:30 PM
Hours: 40 hours per week
Shift Type: Day Shift
Weekend/Holiday Requirement: Every other weekend & holiday

Position Summary

The RN Case Manager is responsible for coordinating patient care, discharge planning, and utilization review to ensure high-quality, cost-effective treatment. This role collaborates closely with physicians, patients, families, and insurance providers to support optimal patient outcomes.

Key Responsibilities

• Coordinate patient care and discharge planning
• Conduct patient assessments and reassessments
• Perform utilization review using InterQual® or similar criteria
• Collaborate with physicians and interdisciplinary teams
• Communicate with insurance providers for authorization and reimbursement
• Identify high-risk patients and connect them with appropriate resources
• Advocate for patients and support informed care decisions
• Facilitate safe transitions to home care or other facilities
• Participate in daily rounds and care planning

Qualifications

• Active Registered Nurse (RN) license in Massachusetts (required)
• Minimum 3 years of acute care experience
• Experience in case management, discharge planning, and utilization review
• Strong knowledge of InterQual® or similar systems
• Bachelor of Science in Nursing (BSN) preferred
• Case Management certification preferred
