Comprehensive Community Services Provider (CCS)

Job description:

Christian Servants Home Care, LLC is a non-medical agency that serves individuals of all ages and abilities. CCS services we provide, include individual skills development and enhancement, employment-related skills training, and parent mentoring, adult and children mentoring. Comprehensive Community Services (CCS) is a program that helps people of all ages live their best lives by focusing on unique needs that relate to mental health and substance use.

We offer paid drive time and flexible scheduling based on your availability.

Job responsibilities include:

• Commuting to and from the client's home or in the community
• Providing 1-1 services to client based on an individualized plan
• Document all face-to-face time using DAP notes
• Work as a part of a inter-disciplinary team, attending team meetings as able
• Communicate effectively with families, service facilitators, and members of the management team
• Actively participate in clinical supervision once a month or every 30 hours of face to face time

In addition applicant must:

• Hold a Master's Degree in the human services field or degree related to the human service field
• Have experience with mental health and substance abuse
• Possess the ability to facilitate meaningful and proactive services to clients undergoing mental health and/or substance abuse recovery.
• Have a cell phone with the ability to clock in and out, track location, and complete
• documentation after each client visit
• Complete 20-40 hours of online training and competency within 90 days of hire
• Complete 8 hours of continuing education annually
• Possess excellent communication skills and response skills
