Registered Nurse (RN) – Home Health (PRN)

What You’ll Do

• Provide skilled nursing care to patients in their homes

• Assess, plan, implement, and evaluate individualized plans of care

• Perform initial and ongoing patient assessments, identifying clinical and psychosocial needs

• Deliver hands-on care and education to patients and caregivers

• Communicate changes in patient condition to physicians and care team members

• Adjust care plans as needed to ensure the best patient outcomes

• Maintain accurate and timely clinical documentation

What You’ll Need

• Current, unrestricted RN license

• At least 1 year of clinical nursing experience

• Home Health or Hospice experience preferred

• Comfortable working independently in a home-based setting

• Willingness to travel within the coverage area

• Reliable transportation, valid driver’s license, and current auto insurance

• Associate or Bachelor’s Degree in Nursing (preferred)

• Experience with HomeCare HomeBase (preferred)

• Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs.
