Job Description

JOB REQUIREMENTS: Safety Planning & Advocacy Work collaboratively with

survivors to develop personalized safety plans for themselves and their children. Conduct ongoing safety assessments and adjust plans as circumstances change. Provide crisis intervention and emotional support in a trauma-informed and survivor-centered manner. Assist survivors in navigating safety concerns related to housing, legal issues, employment, and family dynamics. Develop a group schedule based on safety and well-being of residents- focused on Domestic violence Education and self-care Establish self-care routines with residents in shelter- small incentives for self-care work sheets being completed Plan and coordinate a weekly domestic violence support group, arranging childcare with the Director of Family Services Case Management Conduct intake assessments and develop individualized service plans with survivors. Provide ongoing case management support including goal setting, referrals, and follow-up to healing services Support survivors in making appointments in counseling and finding resource Shelter Support Provide supportive services to residents within the shelter environment. Help maintain a safe, respectful, and supportive shelter community. Support survivors with daily living needs, transitions to permanent housing, and stabilization planning. Documentation & Compliance Maintain accurate, confidential client records and case notes in accordance with agency policies and funding requirements. Track services, referrals, and outcomes for reporting purposes in the Osnuim database Participate in team meetings, case consultations, and program planning. Collaboration & Community Engagement Build and maintain relationships with community partners and service providers. Participate in coordinated community response efforts and referrals. Advocate for survivors\' needs within systems and services. Work with volunteers to schedule groups Develop and implement behavioral management and care plans for clients. Keep records or prepare reports for owner or management concerning visits with clients. Submit reports and review reports or problems with superior. Interview individuals or family members to compile information on social, educational, criminal, institutional, or drug history. Provide information or refer individuals to public or private agencies or community services for assistance. Advise clients regarding food stamps, child care, food, money management, sanitation, or housekeeping. Observe clients\' food selections and recommend alternate economical and nutritional food choices. Observe and discuss meal preparation and suggest alternate methods of food preparation. Consult with supervisor concerning programs for individual families.
