Job Title: Registered Nurse – Case Manager
Location: Anchorage, AK
Type: Contract
Shift: Day Shift (7:00 AM – 4:00 PM or 8:00 AM – 4:00 PM)
Schedule: 5x8 Hours (Every Other Weekend Required)
Weekly Hours: 72 hours per 2 weeks

Position Overview:
Navitas Healthcare, LLC is seeking Registered Nurse (RN) – Case Manager for an exciting Travel or Local job in Anchorage, AK.

Responsibilities:

• Coordinate and manage patient care plans across continuum of care
• Perform patient assessments for care level and discharge planning needs
• Collaborate with interdisciplinary teams to optimize patient outcomes
• Facilitate discharge planning and post-acute care coordination
• Monitor patient progress and update care plans as needed
• Ensure compliance with hospital and regulatory guidelines
• Document and track care using Epic EMR
• Advocate for appropriate utilization of healthcare services
• Communicate with patients, families, and care teams
• Support quality improvement and care management initiatives

Requirements:

• Active Alaska RN license (required)
• Minimum 1 year acute care RN experience
• Epic EMR experience required
• BLS certification required
• Strong clinical assessment and care coordination skills
• Ability to work in fast-paced hospital environment
