East Bay Community Action Program (EBCAP) is seeking a full time Intake Coordinator for our Health Center at 100 Bullocks Point Ave, Riverside, RI.
The Intake Coordinator serves as the first point of contact for patients accessing EBCAP Health Center services, both in-person and via telehealth. This position is responsible for comprehensive intake procedures including patient registration, screening, assessment, and coordination with the healthcare team to ensure patients receive appropriate and timely care.
- Work with health center clients and perform various screenings to determine client needs with respect to healthcare and/or social determinants.
- Refer to appropriate services internally and externally as needed.
- Provide specific assessments to Health Center clients to determine individual client needs. Assessments may include, but are not limited to social history, PHQ (patient health questionnaire), Cage Questionnaire (“cut, annoyed, guilty, eye” – identify problems with alcohol), Tobacco Usage, GPRA (“government performance & results modernization” – collect, improve, report outcomes), Social Determinants, etc.
- Minimum of a High School Diploma or equivalent required.
- Strong commitment to providing quality customer service while handling confidential information in a highly professional manner.
- Excellent telephone skills and a courteous and helpful manner are essential
