Nurse Case Manager II - Michigan

THIS ROLE IS REMOTE HOWEVER MUST RESIDE IN MICHIGAN IN ONE OF THE LISTED COUNTIES TO BE CONSIDERED FOR THIS ROLE. (Berrien, Van Buren, Cass, Kalamazoo, St. Joseph, Calhoun, Branch, Jackson, Hillsdale, Livingston, Washtenaw, Lenawee, Monroe, Oakland, Macomb, Wayne, Clinton, Eaton, or Ingham)

• Monday through Friday
• 8:00 AM - 5:00 PM
• PAY RATE IS $44.00 PER HOUR

Summary

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.

Requirements

• An Associate's degree in Nursing
• An active and unrestricted RN license in the state of MI
• Case Management Certification CCM preferred
• 3 years Clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care
• 1 year of experience managing chronic conditions, such as diabetes, hypertension, and asthma
• Experience preferred managing the CSHCS population (Children's Special Health Care Services)
• Proficiency with computer skills which includes navigating multiple systems and keyboarding
