Job Overview

A healthcare organization in Massachusetts is hiring an experienced RN Hospice Case Manager for a travel assignment in the South Dennis, MA. This role involves providing hospice nursing care and case management services to patients in homes and healthcare facilities.

Responsibilities

• Provide hospice nursing care and case management
• Complete patient assessments and monitor condition changes
• Develop and follow patient care plans
• Coordinate care with physicians and hospice team members
• Educate and support patients and families
• Complete admissions, charting, and documentation on time
• Supervise Hospice Aides as needed
• Maintain compliance with hospice and nursing standards

Requirements

• Active Massachusetts RN License
• Minimum 2 years of Hospice RN Case Management experience
• Hospice Home Care experience required
• Strong communication and documentation skills
• Ability to work independently

Preferred

• IV Certification
• BSN
