• Monday – Friday
• 9:00 AM – 5:00 PM
• 35 Hours Per Week

Job Summary

A hospital-based healthcare facility is seeking an experienced Licensed Master Social Worker (LMSW) to support inpatient medicine, Med/Surg, and rehabilitation patients. The LMSW will provide psychosocial assessments, supportive counseling, discharge planning, and care coordination in a collaborative clinical environment.

Responsibilities

• Conduct psychosocial assessments and supportive therapy
• Coordinate discharge planning and community referrals
• Work closely with physicians, nurses, and care teams
• Maintain accurate patient documentation
• Support patients and families throughout treatment

Requirements

• Active NYS LMSW License required
• Master’s Degree in Social Work required
• Minimum 1 year of LMSW experience required
• Hospital experience required
• Med/Surg or rehab experience preferred
• Strong English communication skills required
