Engineer health

Job Description

Xenios Technologies, Inc. · Remote (US) · Contract → Full-Time · Paid + Founding Equity

The work. Xenios is building the AI-native operating system for the proactive-health professional. Three commercial surfaces. One ontology. One regulated clinical chain. The professional pays. Their clients live inside it. One coach now serves 200 clients instead of 20.

The platform is event-driven and agentic. Five input types — messages, heartbeats, crons, hooks, webhooks — feed a task queue processed by specialized AI agents with persistent state. Architecture is settled. Modules need owners.

The founding engineer owns a major module end-to-end. From a blank repo to live production. Modules include:

  • Agent orchestrator — 15,000+ daily tasks across specialized agents, persistent state, tool routing
  • Coach personality system — clones a professional's voice and judgment into structured prompts and reusable agent definitions
  • Client experience platform — the consumer-side companion layer
  • Real-time form-feedback pipeline — MediaPipe BlazePose, on-device, 30 FPS
  • Multi-channel communication engine — in-app, SMS, WhatsApp, email, with safety guardrails and escalation logic
  • Migration agents — pulling working coaches off Trainerize, TrueCoach, Healthie, Everfit in under 72 hours
  • FHIR R4 clinical surfaces — Patient, Observation, MedicationRequest, CarePlan, DiagnosticReport for the enterprise tier

About the team. Founding team includes a CTO with two prior healthcare-tech exits totaling $710M+, an MD cofounder with 30,000+ patient prescribing history and clinical leadership through a $1.6B SPAC, and a founding bench spanning performance science, clinical operations, product, and operator-side experience across coaching, healthtech, and consumer. Pre-seed with named angel capital closed and MVP build active right now.

How the team builds. Build-to-Learn philosophy. Problem brief → proof of concept → domain expert review → refined spec → production. Working prototypes are specs that run. If the instinct is to wait for a perfect PRD before opening an editor, this is not the right seat. If the instinct is to turn ideas into production systems on Day 1, this is the right seat.

Founding engineers write machine-ready PRDs (working POC code + acceptance criteria + schemas + verification plans). Run Discovery Builds (research a domain, validate with a domain expert, ship). Ship Feature Sprints (well-scoped features in 1-3 days using Claude Code). Communicate asynchronously — learning logs over status meetings.

Stack. Go + TypeScript backend. Supabase + PostgreSQL + pgvector + row-level security. React + Next.js 16 + Tailwind v4. React Native (Expo). Anthropic Claude (Opus 4.7 / Sonnet 4.6) + MCP server architecture. Deepgram + MediaPipe BlazePose. Vercel + Fly.io. Twilio, Stripe + Stripe Connect, Resend, SendGrid. Apple HealthKit, Google Health Connect, Terra. Cursor + Claude Code in the dev workflow with a living CLAUDE.md.

Who fits. Built a production-grade platform from scratch — blank repo to live system serving real users. Thinks in systems: scalability, reliability, security at every layer. Fluent in AI-assisted development without losing engineering rigor. Security-first: HIPAA, SOC 2, OWASP Top 10 from Day 1. Database-first: models in Postgres with real schemas, constraints, and migrations. Tackles the hardest 10% of every problem first — auth, encryption, audit trails, load reliability.

Bonus: agentic AI platforms in production. Health, fitness, or clinical product experience. Supabase / Fly.io / Vercel. HIPAA / SOC 2 frameworks. Real-time video or computer vision. Prior founding-engineer experience.

Compensation. Competitive market-rate cash compensation, paid in 4-week cycles from start date. Founding-level equity grant with 4-year vest, 6-month cliff. 30-day reassessment after start. Final cash terms set at offer based on scope and time commitment. Full-time transition available as the company scales.
