Location: New York, NY 10016
Contract Duration: 14 Weeks
Schedule: Monday – Friday | 9:00 AM – 5:00 PM

Job Summary

"Navitas Healthcare, LLC" is seeking an experienced Licensed Master Social Worker (LMSW) for a 14-week contract opportunity in a hospital setting in New York, NY. The ideal candidate will have experience working in inpatient medical, surgical, or rehabilitation environments and possess strong assessment, counseling, and discharge planning skills.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments for patients and families
  • Provide supportive counseling and therapeutic interventions
  • Develop and coordinate discharge plans to ensure continuity of care
  • Collaborate with interdisciplinary healthcare teams regarding patient treatment plans
  • Facilitate referrals to community resources, rehabilitation programs, and support services
  • Maintain accurate and timely documentation in accordance with facility guidelines
  • Support patients through crisis intervention and care transitions

Requirements

  • Minimum of one (1) year of LMSW experience required
  • Prior hospital experience strongly preferred
  • Experience in inpatient medicine, med/surg, or rehabilitation settings preferred
  • Master’s Degree in Social Work required
  • Active Licensed Master Social Worker (LMSW) license issued by NYSED required
  • Strong communication and interpersonal skills
  • English proficiency required (speak, read, and write)
