Job Type: 13-Week Contract
Shift: 12-Hour Day Shifts | 36 Hours per Week
Pay Rate: $62.00 – $71.00 per hour

Minimum Requirements

• Active Vermont & New Hampshire RN License OR Compact RN License Required
• Graduate of an Accredited School of Nursing
• Minimum 2 Years of Home Health Experience Required

Job Summary

MTK Healthcare is seeking an experienced RN Case Manager – Home Health for an exciting opportunity with Visiting Nurse and Hospice for Vermont & New Hampshire. The ideal candidate will provide comprehensive patient care, develop individualized care plans, coordinate interdisciplinary services, and support patients and families within home health settings.

Key Responsibilities

• Develop and manage individualized patient care plans
• Coordinate care with physicians, caregivers, and interdisciplinary teams
• Perform patient assessments and ongoing evaluations
• Document treatment plans, progress notes, and clinical updates
• Educate patients and caregivers regarding medications, self-care, and treatment plans
• Communicate patient condition changes to healthcare providers promptly
