Job Purpose:
The primary purpose of this position is to assist individuals and/or their families with implementing their self-direction budgets by attending meetings and ensuring payments are made according to budget. As employer of record, ensure quality and Medicaid Compliance standards are met according to regulations. This role also specializes in management of housing subsidies through Office for People with Developmental Disabilities (OPWDD).

Hiring Rate $22-$26/hour commensurate with experience

Essential Functions:
Self-Direction Coordinator
  • Direct contact for person supported and manager of service; this includes serving as a member of their Circle of Support (COS) as assigned per caseload.
  • Provide ongoing training and support to persons supported and COS to gain and maintain the skills necessary to manage self-directed services.
  • Participation in annual and/or semi-annual Life Plan meetings per person supported preference (in person or virtually).
  • Management of compliance related documents assures the timely and accurate completion of plan for individuals receiving services per OPWDD and other regulatory requirements including but not limited to, annual forms, staff action plan, Life Plan, budget and all other supporting documentation.
  • Maintain documentation standards in electronic health records systems.
  • Perform duties associated with the agency’s employer of record responsibilities. This includes assisting individuals with managing their staffs job assignment, wage/status, and onboarding.
  • Perform at minimum, quarterly check-in calls to ensure satisfaction with services.
  • Monitor budget activity to ensure expenses are within the Personal Resource Account (PRA) and meet all OPWDD regulations.
  • Remain compliant with all regulations and guidance associated with self-directed services (including but not limited to; quarterly Webex, Administrative Directive Memorandum (ADM) and Self-Direction Guide for Providers).
  • Reporting any incidents or compliance concerns to appropriate personnel.
  • Consult with Program Managers/Supervisors to ensure consistency of services.
  • Performs other duties assigned by Self-Direction Supervisor and/or designee within scope of practice.
Housing Coordinator:
  • Ensure housing payments and billing meets Federal, State and local regulations, while engaging in continuous learning and being up to date on changing regulations.
  • Handles correspondence and collaboration with people supported, vendors, and support team members in a timely and professional manner.
  • Assists in maintaining and tracking all documents needed for person to receive their housing subsidy.
  • Completion of Housing report and confirmation of payments monthly.
  • Completes and submits monthly housing administrative billing per ADM.
  • Complete in-person inspections of individuals home/apartment to complete Quality Assurance checklist per the ADM.
Non-Essential Functions:
  • Participate in various EPI committees.
Compliance:
  • Must maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to board/vendors/donors’ names, investigations, finance, operations, and employee relations matters.
  • Must complete specific company required trainings as mandated by our governing agency, OPWDD, at time of hire and annually thereafter.

Education and Experience:
  • Bachelor’s Degree preferred in a related field and two years’ experience in the human service field OR a 2-year degree in a related field and 4 years’ experience in the human service field OR High School Diploma and 5 years’ experience in the human service field.
Supervision:
  • None
Computer Skills:
Strong working knowledge of Microsoft programs to include Email, Word, Excel, PowerPoint, Teams.

Qualifications:
  • Must possess and maintain a valid NYS Driver’s License- a vehicle is required for business travel.
  • Mature interpersonal/professional skills to effectively collaborate, resolve issues and maintain confidentiality.
  • Solid verbal and written communication skills and ability to communicate with all employees & self-hired staff within various levels of the organization.
  • Strong numerical comprehension.
  • Strong attention to detail.
  • Demonstrated ability to use poise, tact, and diplomacy, during difficult and/or stressful situations.
  • Proven ability to work in a fast-paced environment, with demonstrated ability to manage multiple tasks, and customer requests, while meeting deadlines.
