Physical Therapist – Outpatient Contract Assignment

Job Title: Physical Therapist
Location: Hyannis, Massachusetts
Contract Duration: 13 Weeks

Position Overview

"Navitas Healthcare, LLC" is seeking an experienced Physical Therapist for an outpatient rehabilitation setting in Hyannis, MA. The Physical Therapist will evaluate patients, develop treatment plans, provide therapy services, and help patients restore mobility and function while preventing disability.

Key Responsibilities

  • Evaluate new patients and establish measurable treatment goals

  • Develop and implement individualized treatment plans

  • Perform diagnostic muscle, nerve, joint, and functional ability testing

  • Administer therapeutic treatments and modify care plans based on patient progress

  • Document patient care, education, and treatment outcomes accurately

  • Communicate with physicians and healthcare staff regarding patient progress and discharge planning

  • Educate patients and families on therapeutic exercises and procedures

  • Supervise and guide Physical Therapist Assistants and support staff

  • Maintain compliance with departmental policies, infection control, and safety standards

  • Participate in meetings, training programs, and continuing education activities

  • Maintain daily patient activity and charge records

  • Perform additional duties as assigned

Requirements

  • Bachelor’s Degree in Physical Therapy from an accredited program

  • Active Massachusetts Physical Therapist License

  • Current BLS Certification

  • Strong communication and patient care skills

  • Ability to work effectively in an outpatient rehabilitation environment

Preferred Qualifications

  • Specialist Certification (for PT Clinical Specialist roles)

  • Experience mentoring staff and assisting with rehabilitation program development
