Job Title: Licensed Master Social Worker (LMSW)
Location: New York, NY
Type: Contract
Shift & Schedule: Day Shift | Monday–Friday | 9:00 AM – 5:00 PM | 35 Hours/Week

Position Overview:
Navitas Healthcare, LLC is seeking Licensed Master Social Worker (LMSW) for an exciting Travel or Local job in New York, NY.

Responsibilities

  • Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, and environmental needs upon admission

  • Provide supportive counseling, crisis intervention, and emotional support to patients and families

  • Coordinate safe and effective discharge planning for patients across inpatient units

  • Facilitate referrals to community resources, home care services, long-term care facilities, and outpatient programs

  • Collaborate with physicians, nurses, therapists, and interdisciplinary healthcare teams to support patient care goals

  • Maintain accurate and timely documentation of assessments, interventions, and discharge plans

  • Monitor patient progress and address barriers impacting discharge and continuity of care

  • Advocate for patients and families to ensure access to appropriate services and resources

  • Participate in care conferences and interdisciplinary meetings as required

Requirements

  • Master’s Degree in Social Work (MSW) from an accredited institution required

  • Current Licensed Master Social Worker (LMSW) License issued by the New York State Education Department (NYSED) required

  • Minimum 1 year of LMSW experience required

  • Strong psychosocial assessment, discharge planning, and case management skills

  • Experience coordinating referrals and community resource services

  • Excellent communication, interpersonal, and documentation skills

  • Ability to work effectively in a multidisciplinary healthcare environment

  • English proficiency required (read, write, and speak)

Preferred Qualifications

  • Acute care hospital experience preferred

  • Experience managing inpatient Medicine, Surgery (Med/Surg), or Rehabilitation caseloads preferred

  • Strong knowledge of discharge planning, care coordination, and community resource networks preferred
