OVERVIEW: F.T. position for Youth Community Support Provider (case managment) with comprehensive insurance benefits eligibility for generous paid leave, a retirement plan with employer match, EAP, and productivity bonus. Direct services to clients and families, including, but not limited to, needs assessment, treatment planning, skill development, service monitoring, referral and linkage, interagency networking, advocacy and crisis prevention. Requires work in clients’ homes, within the community, and in schools. Staff must be able to deliver services in person, by phone, and via an agency-approved telehealth platform, based on client or family preference. Responsible for meeting established productivity standards, including billable service units, in alignment with agency financial goals.
