About The Initiative ProCare's Comprehensive Primary and Community care Teams (CPCT) model has been successfully operating for 20 months. This initiative integrates allied health professionals into general practice teams to improve equity, reduce barriers to care, and support our most at-risk communities. The programme provides free access for patients to a range of services, easing pressure on GPs and enabling holistic, whānau-focused care. About The Role The Practice Pharmacist will serve as the subject matter expert within the practice on all matters relating to medicines information and advice.
