The Home Health RN Case Manager is responsible to plan, organize and direct the Lifespark services while ensuring compliance with regulatory requirements and Lifespark best practices including but not limited to completing, starts of care, care plan development and ongoing support, client satisfaction, caregiver supervision and engagement and on-boarding.
