Job Description The Assessment Registered Nurse will be responsible for conducting regular in-person visits with clients and caregivers in a qualified setting to ensure clients remain safe and healthy in the community while decreasing/preventing unnecessary hospitalizations and re-hospitalizations. They will work closely with the interdisciplinary Care Team in developing proactive action plans to best serve the client's needs. Day-to-Day Responsibilities: Complete intake and annual assessment and admission visits. Complete all required documentation including client assessment forms and multidisciplinary plan of care and communicate professionally with client PCP and other healthcare providers when necessary. Review all information in the client's admission binder, ensuring understanding and educating the caregiver on use of the MH required caregiver communication tool or electronic visit verification system depending on program participation....
