Overview Catholic Charities is seeking a full-time Health Home Care Coordinator for our Care Coordination Services Department through CHHUNY, Children's Health Home of Upstate New York. The ideal candidate will coordinate both the recruitment of children identified with chronic physical and behavioral health conditions and the delivery of person-centered culturally sensitive New York State Health Home Care Management services for youth and families within Western New York. Care Managers work as part of a multidisciplinary team of providers to ensure youth and families have access to needed services to increase wellness self-management and reduce avoidable emergency room visits and/or hospitalizations.
