Job Details Virtual Primary Care/Telehealth Provider Job Description Jefferson Primary Care is currently accepting candidates for the position of Supplemental Access physician, to provide telehealth-based care in service to patients of the primary care network. This position is for a full-time physician to provide telehealth services. It is a unique opportunity to complete outreach to patients who will benefit from a primary care visit and reconnection to a medical home. The Supplemental Access physician will work closely with primary care and population health teams to connect with patients who require same or next day day primary care access, have been out of care longer than recommended intervals and/or who require care at critical touchpoints such as transitions from hospital discharge.
