The Community Transitional Program in Walpole, MA is seeking a qualified candidate to provide direct care & counseling services to clients to prepare for more independent living; provide supervision to staff; supervise day to day operations of residence; coordinate activities for clients; oversee implementation of medication/treatment/rehabilitation/recreational programs; document treatment plans and progress notes; and perform other related duties as required.
