As the RNS 1 specializing in Wound and Diabetic care, the incumbent will assume a leadership role across all five counties in the Taconic region, delivering direct nursing care to individuals requiring advanced clinical support. Collaborating with the Article 16 Clinic team, Nutritional Services, and other specialists, the RNS1 Certified Wound Specialist will assist physicians in the diagnosis and assessment of pressure skin injuries. An essential aspect of this role includes communicating medication changes and condition updates to the team. Collaboration with external healthcare providers, hospitals, and rehabilitation facilities will also be necessary to ensure that treatment is completed and followed through. Individualized wound care plans will be developed for each resident.PRIMARY DUTIES AND RESPONSIBILITIES: Duties include but are not limited to: • Assist physicians in diagnosis and assessment of pressure skin injuries. • Be responsible for recommending and developing individualized short- and long-term treatment plans and monitoring strategies. • Participate in care plan meetings, family meetings, and interdisciplinary team (IDT) meetings. • Attendance at ITT and Life Plan meetings, along with collaboration with dietitians, will be required.• Provide education on diabetic management, encompassing glucose monitoring and insulin administration, health promotion, disease prevention, and self-management to residents, staff and families.• Monitor for preventive long-term complications associated with neuropathy, nephropathy, and retinopathy.• Therapeutic interventions will be implemented to ensure optimal care in the treatment and prevention of pressure-related wound injuries.NOTE: You must have a valid driver’s license to apply as travel is a part of the duties involved in this position.
